Federal Occupational Safety and Health (OSHA) investigators have cited a private contractor, Francis J. Palo Inc., for serious violations and oversights which may have led to the collapse of a 103-year old bridge in Elk County, Pennsylvania during its $2.2 million renovation and replacement. The bridge work has been completed, but an investigation of the collapse is ongoing.
